Do you need a fresh world for Yagluth?

Not on its own. Progress is tracked per character rather than per world, so anything you have already done in another world still counts towards it.

Does world difficulty matter for Yagluth?

No. Yagluth is set to accept any difficulty, like almost every achievement in the game — only three carry a world-difficulty requirement. Whatever settings your world was created with, this one can still be earned.

Why was Yagluth hidden until I earned it?

It is flagged secret, so the game shows neither its name nor its condition until it fires. That is a presentation choice rather than a difficulty one: the requirement is the same whether you can see it or not, which is why it is written out in full on this page.

What do you get for Yagluth?
