Grave generates in Swamp, within 10,000 m from the centre of the world, with 200 placement attempts per world. Worlds are seed-generated, so that band is the answer to "where" — it holds for every seed, which a marked map does not.
How it is placed
The count is placement ATTEMPTS per world, not a guarantee: the generator gives up on a spot that fails the biome, altitude or spacing test, so a cramped world ends up with fewer.

It belongs to Swamp, has to land within 10,000 m from the centre of the world, and the generator makes 200 attempts at it. Where is Grave in Valheim?
In Swamp. There is no fixed position — worlds are generated from their seed — and this one carries no distance restriction, so it can turn up anywhere the biome does.
How many Grave are in a Valheim world?
The generator makes 200 attempts. Each one can fail the biome, altitude, spacing or terrain check and be thrown away, so that is a ceiling rather than a count — a world short on the right biome ends up with fewer.
Which biome does Grave spawn in?
Swamp. Biome is only half the rule though: this one also has to sit between 1 and 1,000 in altitude, so the right biome at the wrong height produces nothing. That is why a location can feel absent from a region you have walked end to end.
Why does Grave appear in different places?
Because its placement rules number 1, and each one can name its own biome, distance band and altitude range. The world generator tries them in turn, so two copies of the same location can sit in terrain that looks nothing alike — they matched different rules, not the same one twice.
