How to get Nephtis Superioris

Nephtis Superioris is the all-achievements unlock: it pops on its own once the other 38 are in. Nothing else is needed, so the real work is making sure none of the 38 slips away.
The ten boss achievements and Birdwatcher come with the campaign. The six side-quest achievements need their quests finished: An Unheard Farewell (Valley of Ashes), The Deserter (Vale of Wrath), The Curious Case of Seamus Diggory (Cursed Hollow), Laudanum (The Outskirts), Monster Hunt (City of Orphans) and The Oathbound, whose seven emblems come off Eternal Guards and whose hidden chamber is in the castle. The five collection achievements want all 29 Ambers, all nine weapons, and one Blade, one Firearm and one Transmutation at maximum level.
Plan the 16 combat feats around the weapons that enable them. Scattershot and Abyssal Eviction are the rarest: Scattershot needs a softened crowd and a point-blank Blunderbuss shot, and Abyssal Eviction needs Shield Bash, which only unlocks at Shield Transmutation level 5. Jackknife needs all nine weapons, so it can only be done after the University. Napoleon's Prison and Entropy have no fast travel, so clear everything you can reach by Lantern before you enter Napoleon's Prison.
