Data: Steam build 25907482 (1.0 launch). How we source this
Missable achievements
Game dataThese can be lost on a playthrough. The rest stay available as long as you can reach them, but Napoleon's Prison and Entropy have no fast travel, so finish the earlier regions first.
Gun of Glory · 1.1% of Steam playersOnly two Masterwork Firearm Parts exist, in City of Orphans and the University, and you need one. Pick it up before Napoleon's Prison: neither the Prison nor Entropy has fast travel back to those regions.
How to avoid missing it →
Statue of Insanity · 0.8% of Steam playersEnraged elites never respawn after dying. If the Cursed Hollow Cannonhand is Sergeant Dubois, killing him before he is fully frozen loses the chance on that playthrough, so fill his Stasis meter before you finish him.
How to avoid missing it →
Transmutation Artifex · 0.7% of Steam playersBoth Quintessential Transmutation Materials, in the University and the Fortress, are in regions you can't fast-travel back to from Napoleon's Prison or Entropy, so pick one up before you go in.
How to avoid missing it →
Ambersmith · 0.6% of Steam playersOne Amber can be lost to how a quest ends. An Unheard Farewell has two reward branches in the game data: one pays Lucky Shot, the other Sleight of Hand. Lucky Shot is also in three chests, but Sleight of Hand has no other source, so Ambersmith needs the branch that pays Sleight of Hand. The data doesn't label which ending pays which, so if you finish the quest and receive Lucky Shot, Sleight of Hand is not available on that playthrough.
How to avoid missing it →
Frenzied Flame · 0.2% of Steam playersDo it before Napoleon's Prison. Neither the Prison nor Entropy has fast travel, so there is no Lantern route back to the Fortress afterwards.
How to avoid missing it →
We Wear the Same Colours · 0.1% of Steam playersThe Oathbound ends in the castle, and Napoleon's Prison and Entropy have no fast travel. Our data can't tell which Eternal Guards carry the seven emblems, so kill every one you meet up to and including the Fortress, finish the quest there, and only then move on: once you are in the Prison there is no Lantern route back to a Guard you skipped.
How to avoid missing it →
The Book of Beasts · 0.1% of Steam playersCity of Orphans has fast travel, so you can come back for missing drawings, but not from Napoleon's Prison or Entropy, which have none. Finish the hunt before Chapter XI.
How to avoid missing it →
Nephtis Superioris · 0% of Steam playersNephtis Superioris itself can't be missed, but some of the achievements it needs can be lost on a playthrough. The clearest case in the game data is Ambersmith: the Sleight of Hand Amber has exactly one source, one of the two reward branches of An Unheard Farewell (the other branch pays Lucky Shot instead).
How to avoid missing it →

Bladesmith · 1.2%Upgrade a single Melee Weapon to maximum. Taking the Saber to level 10 costs:
4Combat challenges (16)
Feats you have to set up on purpose, rarest first. Where the game data says something useful about one, it is noted.
Scattershot · 0.1%Kill 5 enemies with a single Blunderbuss shot. The Blunderbuss deals 15 damage per pellet at level 1 (30 at max), and a whole shot caps at 150 (350 at max), spread over everything it hits; it holds 2 shots. Five kills need frail targets packed together, such as Spewer (50), Risen Musketeer (70), Shambling Civilians (80), Howler (100), Risen Soldier (100) (base Health).
Abyssal Eviction · 0.1%Kill an enemy by pushing them into the abyss with Shield Bash. Shield Bash belongs to the Shield Transmutation, picked up in University.
Jackknife · 0.1%Deal damage with every Melee Weapon, Firearm, and Transmutation without dying. That is all 9 weapons: 3 blades, 3 firearms and 3 Transmutations, each landing a hit in one life.

In the Nick of Time · 35.6%Interact with a Lantern while below 20% Health and with no Flasks remaining.
Mind the Gap · 35.9%Die by falling into the abyss.
